Well-informed healthcare practitioners who understand methods of effective patient communication and collaboration with patients and other clinical team members are in the best position to improve outcomes for their patients with heart failure. Through this comprehensive curriculum, which includes both an on-demand, internet-based audiovisual activity and expert-led, live AAFP chapter meetings, we provide family and general practitioners with the tools they need to manage patients with heart failure throughout the disease continuum. Using a variety of approaches, we will provide participants with the latest information from the heart failure management guidelines from the American College of Cardiology/American Heart Association, key efficacy and safety data on treatment modalities, and information on collaborative care designed to improve adherence and prevent decompensation.
This activity has been designed to meet the educational needs of family practitioners, general practitioners, and other clinicians involved in the management of patients with heart failure.
